Food permit steps
The permit steps,
in order.
- Submit a full set of plans (physical copies only, scaled 1/4 inch per foot, with equipment specification sheets and plumbing, lighting and finish schedules).
- Submit a completed Food Establishment Permit Application, the full menu, and the Risk Assessment.
- Pay the plan review fee (750 dollars).
- After construction, schedule and pass a Pre-Opening Inspection (paid fees allow up to two on-site inspections).
- Pay the operating permit fee by risk category; apply through the Accela portal.
Print it and tick each step as you finish it, so nothing is missed on inspection day.
Plan review
Send your plans
before you build.
You must send a full set of plans to get a food establishment permit. If construction starts before the plans are approved, the department charges a site review fee in place of the plan review fee. A remodel of less than half of the place may qualify for a limited plan review.

Trucks and home kitchens
Not a restaurant?
Ask first.
Food trucks and carts
Mobile food businesses follow Utah Rule R392-102, not the restaurant rule, so ask the department for its food truck steps.
Home kitchens
Davis County lists a microenterprise home kitchen (MEHK) permit. Utah Code 26B-7-416 says a local health department "may grant" one, so confirm the steps with the department.
Each guide shows the rule behind a check, so you know why it is asked.
Food handler permits
Every worker needs one
within 30 days.
Classroom training in Spanish at 22 S State St, Clearfield, 2nd Friday of each month 9 a.m., call 801-525-5120 to reserve a seat; picture ID required. Otherwise take a Utah-approved online course; the vendor certificate is valid for up to 30 days while the permit is processed by the health department where you live. Food safety manager documentation is kept at the establishment and entered into a health department database.
Permit fee: 25 dollars, as published on September 29, 2026.
Utah Code 26B-7-413: finish approved training within 14 days of starting, and get the food handler permit within 30 days.
CleanCleaver keeps each worker's 14 and 30 day dates on one board, so nobody's permit slips.
City business licenses
The city license
is a separate step.
Layton
Layton requires a valid city license for all businesses. New applicants create an account in CityInspect. Each license ends one year after it is issued. City business license page (read September 29, 2026).
The checklist puts the city license and the health permit in one order.
