Food permit steps
The permit steps,
in order.
- Choose the permit type: permanent food establishment, mobile food truck, or temporary (1-2 events, or 3+ events per year as an annual permit).
- Submit the application through the department's online portal (applications linked from the Food Services page).
- Have plans reviewed (fee schedule lists 'Review restaurant plans').
- Permanent establishments and mobile food trucks must have at least one certified Food Safety Manager; food workers need a Utah Food Handler Permit.
Print it and tick each step as you finish it, so nothing is missed on inspection day.
Plan review
Send your plans
before you build.
The fee schedule lists a plan review with a two hour minimum. The department's food page does not describe the plan review steps in words, so call the office before you start. The Moab office number is in the contact list below.
Forms and guides from the department
The checklist shows what an inspector can check, so your plans match it.
Fees
What the department

See the daily checks that fill your binder while you wait on your permit.
Trucks and home kitchens
Not a restaurant?
Ask first.
Food trucks and carts
Mobile food businesses follow Utah Rule R392-102, not the restaurant rule, so ask the department for its food truck steps.
Home kitchens
Southeast Utah lists a microenterprise home kitchen (MEHK) permit. Utah Code 26B-7-416 says a local health department "may grant" one, so confirm the steps with the department.
Each guide shows the rule behind a check, so you know why it is asked.
Food handler permits
Every worker needs one
within 30 days.
Utah Food Handler Permit after an in-person or online state-approved training course (Food Services page).
Permit fee: 20 dollars per fee schedule, as published on September 29, 2026.
Utah Code 26B-7-413: finish approved training within 14 days of starting, and get the food handler permit within 30 days.
CleanCleaver keeps each worker's 14 and 30 day dates on one board, so nobody's permit slips.
City business licenses
The city license
is a separate step.
Moab
The city licenses all businesses inside city limits. Contact the City Treasurer's office at 435-259-5123. Approval can take as little as three days, and up to six weeks for licenses that need City Council approval. City business license page (read September 29, 2026).
The checklist puts the city license and the health permit in one order.
