Food permit steps
The permit steps,
in order.
- Get the forms from the department's Permits & Applications page (Food Service Establishment Permit, Mobile Food Business, Temporary, Seasonal, Intermittent, Food Safety Manager Certification).
- Submit a food establishment plan review for a new establishment or extensive remodel (fee by risk category).
- Pay the annual permit fee by risk category.
- Every restaurant serving TCS foods needs a full-time Certified Food Safety Manager, who must submit a registration form and a copy of the certification to the department.
- All food workers need a Utah Food Handler Permit.
Print it and tick each step as you finish it, so nothing is missed on inspection day.
Plan review
Send your plans
before you build.
The fee schedule lists a plan review for a new food place or an extensive remodel. If construction is already under way, the plan review costs the normal fee plus 100 percent. If construction is finished, it costs the normal fee plus 200 percent. An on-site talk or a small remodel has its own lower fee, listed below.

Trucks and home kitchens
Not a restaurant?
Ask first.
Food trucks and carts
This department posts mobile food forms: Permit to Operate a Mobile Food Business. Mobile food businesses follow Utah Rule R392-102, not the restaurant rule, so ask the department for its food truck steps.
Home kitchens
Weber-Morgan lists a microenterprise home kitchen (MEHK) permit. Utah Code 26B-7-416 says a local health department "may grant" one, so confirm the steps with the department.
Each guide shows the rule behind a check, so you know why it is asked.
